Really looks to me that the side plate wasn't fitted properly, the pins don't look peened so it just fell off?
I don't think I would trust a side plate just pressed on, stock DID are a tight press fit, I still peen them

these are suppose to be solid hardened core so they are not suppose to be peened unless Les changed links and I didn't know

I don't think I would trust a side plate just pressed on, stock DID are a tight press fit, I still peen them
You were right got this back from Les. "It is, in fact, "solid" press-fit construction, but it has always required peening (or a cross score) to flair the ends of the link pins. I recommend a cheap pair of bolt cutters with one jaw ground flat to affect the correct depth of cut, and a simple X pattern on the end of the link pins. A heavy-ish steel rod as a back-up "anvil" and a small ball peen hammer works equally well.
