Adjusting Valve shims (Under bucket)

I have done this a couple times before on my KZ650 with no problem, but this last attempt has been chaotic.
When I installed the cam chain tensioner I released the spring and set the tension before I had the crank in the right position. I rotated it to the correct index and reset it – during this time while I was turning it over I heard a few clicks or snaps . When I rechecked the gap the intake was close but the Ex was wonk, a few too tight and a couple very large (.018” instead of .003-.007). I never thought to count off the cam chain links between EX and IN cams so I can’t say for sure but I now think the clicking was the cam chain jumping a tooth or two on one of the sprockets.
Anybody care to chime in?
What is the proper sequence for installing the CCT? I hold the plunger in with a cotter pin (and tightened the lock bolt) until I have the mounting bolts almost tight. Then pull the cotter pin and turn it to proper crank index before loosening the lock bolt. Does that sound correct? Should I loosen the lock bolt as soon as I pull the cotter pin?
